'06 FZ6 Strange Shifting Into 4th Gear

Hi everyone. I've had my '06 Fz6 for about 2 weeks, it has 12k miles, brand new chain/sprocket (stock gearing) air filter, and exhaust. I'm new to the Fz6 but I had a 2007 R6 for 2 years prior. I'm fairly mechanically inclined but this has me stumped..
I would say once out of every 100 times I shift into 4th gear it feel exactly like "missing second gear" and it revs freely for a second before it grabs and then it's fine. The other 99 times I shift into 4th it's smooth as butter. Twice now that it's happened before grabbing back into gear it made kinda a rumbling noise before going to gear...
I used to have a quad that did this all the time and literally for like 4 years it did this constantly but never gave me any trouble in the long run.
If anyone had any idea what this please let me know. I was thinking maybe it was something I was doing wrong but it seems unlikely considering I've ridden probably 10,000 miles on sportbikes in the last 4 years and this has never happened before. Thanks!

I should also include that this only happens shifting UP into 4th and never shifting down into 4th. I usually shift very smooth! The throttle on this bike is a little jerky on the on/off throttle but I rev match really well on downshifts and I make quick and smooth gear changes 95% of the time.
